Allogeneic (donor) multipotent mesenchymal stromal cells (Al-MMSC) from bone marrow

For therapy, we use MMSCs obtained from the bone marrow of young and healthy donors. Compared to one's own (autologous) cells, donor cells have a significantly more pronounced effect on treatment and revitalization.

All donors are healthy young people aged 18-25. Dokar selects donors independently. Donors must not smoke, drink alcohol or use drugs. They must not have tattoos (tattoos significantly reduce the activity of MSCs).

Before collecting bone marrow, donors undergo a full examination of the body. Bone marrow is taken from the iliac bone in an amount of no more than 100 ml. The procedure is performed under local anesthesia (ultracaine) and is painless. The donor feels well.
Pure cells (MMSCs) are isolated from the bone marrow and tested for safety and activity. Then the pure fraction of MMSC is placed in a nutrient medium and grown under certain conditions (temperature, pressure, lighting, concentration of O2 and CO2) for 5-7 days.

Bone marrow (BM) is the only human tissue that contains a large number of regenerative cells. Bone marrow is a reserve supply of stromal cells for natural regeneration and emergency recovery of the body (in case of injuries or acute diseases). There are two types of cells in the bone marrow:
  • Hematopoietic (HPC) - can transform into blood cells (erythrocytes, leukocytes and platelets)
  • Mesenchymal multipotent (stromal, (MMSC)) - can transform into most types of cells.

Main properties of bone marrow MMSCs:

Age and regeneration

MMSC - differences between donor (allogenic) MMSC and one's own (autologous):

1. Donor MMSCs are more active, as they are obtained from younger and absolutely healthy donors.

2. Donor MMSCs have a long-term paracrine effect (the ability to activate one's own local progenitor cells and produce growth factors). One's own MMSCs do not have this effect - they immediately turn into specialized cells to replace damaged ones.

3. Limited period of action - donor MMSCs are active for 30 days, then apoptosis (death of MMSCs) occurs and their remains are utilized by one's own leukocytes.
Security and compatibility of MMSC

1. Compatibility - donor MMSCs do not react with the patient's immune system, so they can be administered to any patient. This is due to the presence of a special protein on the surface of MMSCs - MHC-1. There are no cases of immune incompatibility of donor MMSCs during their therapeutic use.

Is there a risk of rejection?
Donor (allogeneic) mesenchymal multipotent stromal cells (MMSCs) do not react with the immune system and, accordingly, do not cause a risk of allergic reactions or rejection.

MMSCs have one important property - the ability to actively interact and change the activity of almost all cells of the immune system (IS), creating an immunotolerant environment for both the MMSCs themselves and the surrounding cells and tissues.

Immunomodulatory properties allow us to consider MMSCs as a safe means of overcoming pathological inflammatory processes and ensuring immune tolerance in organ transplantation and autoimmune diseases. MMSCs are capable of both directly and indirectly (via other cells of the immune system) influencing immune system cells. MMSCs exert their immunomodulatory action through various mechanisms, including secretion of soluble factors and cell-contact interaction with target cells.

2.Safety. MMSCs are not able to multiply and transform in the body. The use of this type of stem cells began in 1996 and until 2025, not a single negative case of their use has been recorded.

Is there a risk of developing cancer?
In 2013, a group of European scientists published a long-term study of the safety of MMSC cell administration procedures used to treat orthopedic problems. In the period 1990-2006, 1873 patients were treated with MMSCs obtained from bone marrow. Patients were monitored from the time of the first administration in 1990 until December 31, 2011 or earlier deaths. Such a long period (an average of 12.5 years in the range of 5-22 years) helped track the possible development of cancer. The primary outcome was to assess the risk of developing tumors at sites treated with MMSCs. The secondary outcome was to show the risk of being diagnosed with cancer at sites not treated with the cells. After reviewing 7,306 MRI images and 52,430 X-rays from 1,873 patients, not a single tumor was found at sites treated with MMSCs.

The study showed no increased risk of developing cancer over a mean follow-up of 12.5 years.

There is no rational reason for the fear, other than that a completely different type of cell (Embryonic stem cell) has the same name (stem cell) but can cause teratomas - hence the confusion!
